Qualifications for a Medical Marijuana Card in NY:

Step 1: Are You Experiencing Any Of The Following Qualifying Conditions?

Qualifying Conditions​:


A patient can be certified to use medical cannabis in New York state if they suffer from one of the following:

  • Chronic Pain (Arthritis, Migraine, Fibromyalgia, Back Pain, Neck Pain, Severe Dysmenorrhea, Endometriosis, Uterine Fibroids, etc) 

  • P.T.S.D. Post Traumatic Stress Disorder (Anxiety, Depression, Insomnia, Panic Attacks, etc)

  • I.B.D. Inflammatory Bowel Disease, Ulcerative colitis, Crohn’s Disease, I.B.S.  Irritable Bowel Syndrome (Abdominal pain, gas, bloating, cramps, etc)

  • Pain that degrades health and functional capacity where the use of medical marijuana is an alternative to opioid use.

  • Opioid Use Disorder/Substance Use Disorder, you must be enrolled in an Article 32 Treatment program

  • Neuropathy, Radiculopathy, sciatica, pinched nerve, numbness, tingling

  • Epilepsy, Seizure Disorder

  • Cancer

  • HIV or AIDS

  • A.L.S. Amyotrophic Lateral Sclerosis (A.K.A. Lou Gehrig’s Disease)

  • Parkinson’s Disease

  • Multiple Sclerosis

  • Damage to the nervous tissue of the spinal cord with objective neurological indication of intractable spasticity

  • Huntington’s Disease

The associated or complicating conditions include:

  • PTSD

  • Opioid Use Disorder (must be in an Article 32 Treatment program)

  • Seizures

  • Severe Nausea

  • Severe or Persistent Muscle Spasms

  • Severe or Chronic pain resulting in substantial limitation of function

  • Cachexia/Wasting Syndrome

The Medical Documentation Necessary for Eligibility

To ensure eligibility and to begin the patient certification process, we require each patient to have a copy of their medical records to confirm your diagnosis.

The recommended medical documentation should document your qualifying medical condition.  Please include your current medical diagnosis, list of medications, any surgeries, or diagnostic tests confirming your qualifying condition.

After review of your medical documentation, & Photo ID, our staff will contact you to schedule an appointment.

If documentation is not available, please download the HIPAA Medical Release and Fax Cover Letter. Sign and complete and send directly to your Physician, who will then be able to send us your medical records on your behalf.

You will need to provide us with a copy of your valid NYS Drivers License or Non-Driver ID, and a copy of your available medical documentation and/or records prior to your appointment.

Once you receive your certification from us, you will be able to register online for a medical marijuana card with the Department of Health. This card must be shown when buying medical marijuana at a dispensing facility and carried by the eligible patient. ​​​

Proof of Identification & Residency:

You’ll need to provide one of the following items to show proof of residency:

  • New York State Driver’s License or New York State Non-Driver ID Card.

  • Copy of a government-issued identification card that contains your name and New York State address.
